Case Management is defined as a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. The goals of case management are to help the patient to be referred to the right services, reduce hospital admission and readmission and encourage discharge or care plan adherence. There are six steps of case management. They are client identification/selection, Assessment, planning, implementation, evaluation & follow-up and termination of case management process. Firstly, client identification. The case manager will identify client who would benefit from case management services and the client must fit on certain specific criteria. During the client identification service, they will use screening or assessment tool to identify the right client. Secondly, assessment begins after the completion of case identification and intake into case management. Through assessment, it can determine the needs of the patients. Based on the assessment data and findings, case manager can identify the actual and potential problems to address. In addition, the case manager can set goals for the treatment and identify necessarily interventions and determine appropriate resources. Thirdly, planning phase starts. Case manager will establish goals and prioritise the patients’ needs and type of services and resources required to meet the goals. Once the planning is up, case manager facilitates the development of the plan and revisions of plan. There are several community services that help the patients. For example, meals on wheels, laundry services, escort services for appointment and financial assistance. These are some of the services that the case manager can plan for their client.Before implementing all the plans into actions, the case manager would need to conduct a home visit to discuss with his client about his action plans. The case manager has another role in which he must ensure that the client fully understand the treatment plan and they must agree with the plan too. It is important for case manager to plan how to go their client’s house. First impression is key and they should plan properly before visiting the client. After planning, execution part steps in. Once the client has agreed with the plan, the case manager will put the case management plan into actions. Case manager reassess the patient condition on whether if there is any problem in the plan or the plan require some modification. Fifthly, evaluation and follow up. it involves the evaluation of the patient care activities and outcome. The case manager evaluates by examine the patient’s condition and the status of the desired outcome. Case manager can make another home visit to follow up with their client who recently received services from their program. Lastly, termination of case management process. This happens when the case is close. The termination of case is based on patient and family condition or choice. It can be also when the patient has achieved the targeted goals and outcomes. When the case is close, case manager no longer need to provide case management services. (509 words)
